2010 Give Kids A SmilePatient Information

“Give Kids a Smile” is currently helping children in the communities of Allen, Denton, Frisco, Flower Mound, Lewisville, McKinney, Prosper and Plano. The GKAS program is only available to children in need who-because of financial limitations-are unable to access dental care through traditional private practice facilities. In order to qualify, the parents of these children must demonstrate such need through their participating schools. Local school nurses have agreed to assist in the identification and enrollment of underserved children in their schools. Guidelines for qualification can be obtained through their offices.
Local school nurses will be able to register qualified patients and provide all necessary documentation that is required of each patient before assignment to a treating doctor. Parents must complete this information and turn it in to the nurse as soon as possible in order to qualify. Incomplete forms or failure to turn them in by the due date will disqualify the child from participating in the GKAS event.
School nurses will provide the completed registrations to the GKAS Committee. The Committee in turn will use this information to make treatment charts for each patient and then distribute them to the volunteer doctors.
GKAS patients will be directly assigned to doctors for preventative and basic care (availability of volunteer doctors will determine patient assignments). Parents of children assigned to a dentist office will be contacted directly by the dentist office to schedule an appointment date and time for them. Patients assigned to the CCCC Dental Hygiene School (January 30, 2010) or the Texas Women's University Hygiene School (February 20, 2010) will be notified by the school nurse with an assigned time for their child's appointment. It is the responsibility of the parents to be sure that the child is at the doctor’s office on-time for the scheduled appointment. Failure to do so may result in that child being unable to receive dental care.
Children with multiple dental problems will be given a determination of treatment with possible alternatives for treatment. Please remember that this is a once a year event, and the doctor will not be responsible for additional care beyond this appointment day.
